The admission process for becoming a resident at Restored Bridges, LLC involves 3 steps:
Step 1: Check your eligibility and admission requirements for the desired program here. The number of vacancies are also posted.
Step 2: If you meet the requirements for becoming a resident- submit a referral here. Please allow 24-48 hours for an acceptance decision via your preferred method of contact.
Step 3: If you are approved to becoming a resident- please complete the necessary Intake packet here.
After all required documentation is received- the resident will be assigned a Housing Specialist who will assist the resident with the admission and move in process and will support the resident throughout the program.
Yes, Restored Bridges, LLC is a residential program that provides housing and support for individuals who wish to live independently in their communities or transitioning into society. Our residents live on-site and receive comprehensive care and services.

Currently, we only accept Partners Behavioral Health plans for our Independent Living IDD and Outpatient Therapy Programs.
